To support people to take pro-active steps to improve the way they manage their physical and mental health conditions on a 1:1 basis, based on what matters to them. To support people to develop their knowledge, skills and confidence, building their “patient activation” scores to improve their health outcomes and quality of life. This role will also have a community and group delivery focus to improve the number of opportunities that people have to get active in ways that suit them. A Register of Exercise Professional qualification at Level 2 or 3 is highly desirable.

About North Sedgemoor Health and Wellbeing Coach

We are looking to recruit a new Health and Wellbeing Coach in North Sedgemoor to support people to take pro-active steps to improve the way they manage their physical and mental health conditions, based on what matters to them as well as supporting them to develop their knowledge, skills and confidence – or to build their "patient activation" - in managing their health and care, to improve their health outcomes and quality of life. This role will also have a community and group delivery focus to improve the number of opportunities that people have to get active in ways that suit them.

The role is a key part of the Primary Care Network (PCN) multidisciplinary team. You will work alongside Social Prescribing Link Workers and Care Coordinators to provide an all-encompassing approach to personalised care across the North Sedgemoor Primary Care Network.
